ด้านหน้ามีพระบรมสาทิสลักษณ์ พระบาทสมเด็จพระจุลจอมเกล้าเจ้าอยู่หัวและพระบาทสมเด็จพระเจ้าอยู่หัวภูมิพลอดุลยเดชเป็นประธาน ริมขอบเหรียญมีพระราชลัญจกรปรมาภิไธยของทั้งสองพระองค์
ด้านหลังเป็นรูปพญาคชสีห์ในพระธรรมนูญใช้ตรากล่าวไว้ว่า เป็นตราเจ้าพระยามหาเสนาบดีฯ สมุหกลาโหม พยาคชสีห์เป็นสัตว์หิมพานต์เป็นส่วนผสมระหว่างช้างกับสิงห์ กายเป็นสิงห์ หัวเป็นช้าง มีพละกำลังเท่ากับช้างกับสิงห์รวมกัน รอบขอบเหรียญจารึกว่า “๑0๘ ปี แห่งการสถาปนา กระทรวงกลาโหม ๘ เมษายน ๒๕๓๘ ๒0 บาท ประเทศไทย”
