ด้านหน้ามีพระบรมสาทิสลักษณ์ พระบาทสมเด็จจุลจอมเกล้าเจ้าอยู่หัวและพระบาทสมเด็จพระเจ้าอยู่หัวภูมิพลอดุลยเดชเป็นประธาน รอบริมขอบเหรียญจารึกว่า “จุลาลงกรณ์ ปร. ภูมิพลอดุลยเดช ปร.”
ด้านหลังของเหรียญมีตราสัญลักษณ์กรมศุลกากรพร้อมคำจารึก ๑๒๕ ปี กรมศุลกากรและภาพตึกเก่าหอรัษฎากรพิพัฒน์สร้างในสมัยรัชกาลที่๕ และตึกใหม่ในปัจจุบัน รอบริมขอบเหรียญจารึกว่า “๔ กรกฎาคม ๒๕๔๒ ประเทศไทย ๑0 บาท ”
