ด้านหลังเป็นเป็นตราสัญลักษณ์กรุงเทพมหานครและมีคำจารึกว่า“๑00ปี โรงพยาบาลกลาง สำนักการแพทย์ ๒๔๔๑-๒๕๔๑ ประเทศไทย ๑0 บาท”
